  1. Identify the exact document
  2. Separate the entry from your question
  3. Ask where to send the request
  4. Record the answer without guessing
  5. If the service issues a new version

Your next step: Mark the detail you want checked, identify the document it appears in and ask the clinic or lab that issued it for clarification. Keep the original unchanged. Use this sheet to record your question and the reply.

A misspelt name, a date you do not recognise or a note that differs from your recollection can prompt a question. This worksheet does not decide that a clinical entry is wrong, interpret a result or establish a legal right to a correction. Yellow at spotyellow.in does not receive your documents or submit a request. Print the fields or copy them into notes you control.

Identify the exact document

Start with the document title and issuing service. If you have several versions, note which one contains the detail you are asking about.

Clinic, hospital or lab that issued it: ____________________

Document title and date shown on it: ____________________

Page or section containing the detail: ____________________

Where I keep the unchanged original: ____________________

Date I received it, if known: ____________________

Keep your own observations separate from the original report. Do not edit a PDF to make it look as though the clinic issued different wording. If the document seems to belong to someone else, contact the issuing service through its confirmed channel; avoid circulating it to other people.

Separate the entry from your question

Copy only the wording needed to identify the issue. For a long report, a page number and short quotation can keep the request focused.

Detail as written What I want checked or explained Supporting detail I can supply if requested

For an administrative detail, you might write: “The appointment date on this summary differs from my booking confirmation. Please check which date is correct.” For a clinical phrase you do not recognise, ask what it means and whether the practitioner can review your question. An unfamiliar term alone does not establish an error.

Ask where to send the request

Confirm the service's process before sharing a full report or identity document. Use the channel the service confirms and ask what information it needs to locate the record. This sheet does not ask for passwords, one-time codes or identity-document numbers.

Confirmed contact or department: ____________________

Channel the service told me to use: ____________________

Information the service requested: ____________________

Request sent on: ____________________

Reference number, if provided: ____________________

Message draft: “I received ______ dated ______. On page ______, it says ______. Please check this detail or explain it to me. My question is ______. Please let me know who will reply and how I can identify the current version if a corrected document is issued.”

Record the answer without guessing

Write down whether the reply explains the existing entry, confirms a correction or asks for more information. A sent request does not mean the service has agreed that a change is needed.

Who replied, and when: ____________________

Explanation or action stated by the service: ____________________

Where the written reply is saved: ____________________

Question still open: ____________________

Follow-up contact or timing stated by the service: ____________________

There is no response deadline promised by this worksheet. If the uncertainty affects care you need now, contact the treating practitioner or an appropriate medical service rather than waiting for an administrative correction. Do not change a medicine or act on a test result using your own rewritten version of the record.

If the service issues a new version

Record the new document's date and where you saved it. Ask the issuing service how to distinguish it from the earlier version. Keep the earlier document and the reply so you can show what happened if asked.

New version received on: ____________________

How the service identifies the current version: ____________________

Where I saved both versions and the explanation: ____________________

Practitioner already holding the earlier copy, if any: ____________________

How I will confirm they have the current document: ____________________

Use the existing-records index to locate documents. Use the test request question sheet for who will explain a test result. This clarification sheet keeps one apparent discrepancy and its resolution together.
