Your next step: Use this sheet when you or the clinic needs to change a booked appointment. Keep the original booking, your request and the clinic's confirmation in separate boxes. A request for a new date is still a request until the clinic confirms it.
Print the sheet or copy it into your own notes. Yellow at spotyellow.in does not send the request, change your appointment or store what you write. This is a booking record, not advice about how long you can wait for care. If you need medical help sooner, contact a medical service instead of waiting for a booking reply.
Start with the booking you already have
Copy the details from the confirmation message rather than from memory. For a hospital with several branches, include the branch as well as the practitioner's name.
Practitioner and clinic or hospital branch: ____________________
Original date and time: ____________________
In person or video: ____________________
Booking reference, if provided: ____________________
Where I saved the original confirmation: ____________________
Who requested the change, and when: ____________________
If the clinic cancelled, save its message. If you asked to change the time, keep your sent request. Neither entry tells you whether another date has been reserved.
Write the change you want
State which appointment you mean and give the dates or times you can attend. You can keep your reason brief. A booking request does not need your whole health history.
My preferred date or time window: ____________________
Other dates I can attend: ____________________
Location or call format I need confirmed: ____________________
Message draft: “I have an appointment with ______ at ______ on ______. I need to request a different time. I can attend ______. Please confirm the available option and whether my original booking remains in place while this is arranged.”
If you have questions about the medical timing of the visit, ask the clinic how to reach the practitioner for that question. Do not use the next available calendar slot as an answer about whether a delay is appropriate.
Record the request and the reply
Use the clinic's confirmed booking channel. Check the recipient before sending personal details. Write “awaiting reply” when that is the current position.
| Date contacted | Confirmed channel or person | What I requested | Reply received |
|---|---|---|---|
How the clinic says it will confirm the change: ____________________
Contact route if I have not heard back: ____________________
Any response timing the clinic has stated: ____________________
This sheet does not promise a response time or that a message is monitored outside clinic hours. If two messages show different bookings, ask the clinic which one is current.
Close the loop on the new booking
Fill this section from the clinic's reply. Leave unconfirmed details marked “to confirm”.
Confirmed new date, time and practitioner: ____________________
Confirmed branch, address or video link location: ____________________
What happened to the original booking: ____________________
Confirmation received from, and date: ____________________
Where I saved the new confirmation: ____________________
If your appointment had preparation instructions, ask the clinic whether the date change affects those instructions. Record where the clinic's answer is saved. This worksheet supplies no fasting, medicine or test-timing instructions.
Update only the arrangements that changed
Use these boxes for your own calendar and practical arrangements once the booking is confirmed.
- Update my calendar and remove the superseded reminder.
- Check travel or the time set aside for a video call.
- Tell a companion about the new time, if I want their help.
- Keep my questions and existing documents ready for the new visit.
Use the appointment preparation sheet for what you want to discuss. If someone is joining you, the companion sharing checklist helps you agree on their role. This sheet's job is the booking change itself.
