Your next step: Write down the part of your appointment experience you want the clinic to understand, then state the response you would find useful. This sheet helps you prepare feedback about communication, privacy, organisation or practical arrangements.
Print it or copy it into your own notes. You decide whether to send anything. Yellow at spotyellow.in does not receive the completed sheet, investigate a clinic or submit feedback for you. This is a communication aid, not a legal complaint form or an assessment of medical care. Questions about symptoms, treatment or urgent help need a medical contact route rather than a feedback inbox.
Identify the visit without oversharing
Start with enough detail for the clinic to locate the appointment. Check the recipient and the clinic's feedback process before including personal information. Keep private health details out of a public review if you do not want them made public.
Clinic or hospital branch: ____________________
Appointment date and approximate time: ____________________
In person, phone or video: ____________________
Booking reference, if needed by the clinic: ____________________
Person or team I intend to contact: ____________________
You can write this first version for yourself. You do not need to send every detail you record here.
Separate what happened from what you understood
Describe one event at a time. Use the words you remember accurately; if the wording is approximate, say so. Leave another person's motives out of the factual description.
| What I observed or heard | What I understood or felt | What I want clarified |
|---|---|---|
For example, “The call ended before I asked my final question” describes an event. “I left unsure how to ask that question” describes your experience. Both can belong in your note without claiming to know why the call ended.
Anything I am unsure I remember correctly: ____________________
A message or confirmation that helps establish the sequence: ____________________
Save originals separately. Do not edit a clinic message to make it fit your recollection. If your concern is an entry in a health record, use the record-clarification request sheet for that separate purpose.
Name the practical effect
Keep this section specific to your experience. You might have been unable to hear an explanation, unsure who was in a video call, or unclear where to send a follow-up question. Use your own example, not a category you feel expected to choose.
What this meant for me: ____________________
What worked well and I would like retained: ____________________
The one issue I most want addressed: ____________________
You can acknowledge something helpful while asking for something to change. You can also leave the positive-feedback field blank.
Ask for a concrete response
Choose a request the clinic can understand: an explanation of a process, a named contact, confirmation of a privacy arrangement, or a way to submit an unanswered question. The clinic will need to confirm what it can offer.
My request: ____________________
My preferred way to receive a reply: ____________________
Any communication access need I want to state: ____________________
Draft message: “I attended an appointment at ______ on ______. I want to share feedback about ______. What happened was ______. This left me ______. Please let me know ______. You can reply through ______.”
Before sending, remove details that are not needed for the request. If someone helps write the message, read the final version yourself and check what you are agreeing to share.
Record the response without assuming an outcome
Confirmed feedback channel: ____________________
Date sent and reference, if supplied: ____________________
Acknowledgement received: ____________________
Response timing stated by the clinic, if any: ____________________
Reply received and date: ____________________
What remains unanswered: ____________________
An acknowledgement means the message was received, not that the issue has been resolved. If you have no reply, record that plainly and check the clinic's published process for following up. This worksheet does not set a legal deadline or guarantee a response.
Keep your care questions separate
Use the after-appointment action sheet for the next steps discussed in your visit. A feedback request does not change those instructions or replace a clinical conversation.
If you want to learn about Yellow's programme separately, visit Yellow for Hormones. The programme page is not the clinic's feedback channel, an emergency service or an independent complaints body.
This resource provides general information and does not diagnose a condition or replace advice from a qualified practitioner.
