Your next step: If you have decided to arrange a second opinion, write the question you want it to answer before collecting more paperwork. Use this sheet to record explanations in the practitioner's own words and identify anything that still needs clarification.
Print it or copy the fields into your private notes. Yellow at spotyellow.in does not book the visit, choose between opinions or store what you write. This worksheet does not decide whether you need a second opinion or whether it is safe to wait for one. Take questions about timing or changes to your care to the practitioner responsible for that care; seek urgent medical help when needed instead of waiting to complete a sheet.
Name the question you want answered
You may want an explanation repeated, a recommendation discussed in more detail, or an unanswered question addressed. Write that purpose without deciding in advance what the new practitioner should conclude.
The question I want clarified: ____________________
What I understood from the first conversation: ____________________
What I am still unsure about: ____________________
What matters to me in this discussion: ____________________
Use “my understanding” when you are working from memory. Copy a quotation only if you have the actual wording. You do not need to make your notes sound medical.
Confirm what the new appointment includes
Ask the clinic what information it needs and whether the appointment is with the practitioner you expect. A booking confirmation is separate from a promise that a particular question can be resolved in one visit.
Practitioner, clinic and branch: ____________________
Confirmed date, time and format: ____________________
Documents requested by the clinic: ____________________
Where and how the clinic asks me to share them: ____________________
Anything about the visit I still need confirmed: ____________________
If you are still identifying who to contact, Ask Yellow where to start opens our care-navigation guide. It does not book a consultation or assess your case.
Bring existing information with its context
Use your existing-records index to locate documents. Check the name, date and issuing organisation on each item. Keep original reports intact; label your own summary separately. Do not arrange new tests just to fill this worksheet.
| Existing document or note | Date and author, if known | Where saved | Shared with the clinic? |
|---|---|---|---|
Relevant information I do not have: ____________________
What I need to ask the clinic about missing records: ____________________
Avoid adding identity documents or unrelated private records unless the clinic has explained what it needs. Check the recipient before sending files.
Take these questions into the conversation
Choose the questions that fit your purpose. Leave space for the explanation rather than using the sheet as a scorecard.
| Question | Practitioner's explanation | What I want clarified |
|---|---|---|
| What information are you using to form this opinion? | ||
| Which part of my question can you answer today? | ||
| What remains uncertain, and how will it be addressed? | ||
| If this differs from the earlier explanation, can you explain the difference? | ||
| Who should I contact with questions after this visit? |
One point I would like repeated in plain language: ____________________
Keep differences visible
Two notes side by side can help you formulate a question; they cannot tell you which medical recommendation to follow. Do not combine instructions into a new plan yourself. Ask the responsible practitioner to clarify what applies to you and who is coordinating the next step.
Earlier explanation, with date and source: ____________________
New explanation, with date and source: ____________________
The difference I need explained: ____________________
Who I will ask, using which confirmed contact route: ____________________
Response received and date: ____________________
Close with a clear handover
Written summary offered or requested: ____________________
Who is responsible for the next conversation: ____________________
What I have agreed to share, and with whom: ____________________
