Your next step: Use this sheet after an appointment to record what was agreed, who will do it and when to check back. Copy confirmed instructions from the practitioner. Put anything you did not understand in the clarification box instead of guessing.
This is a personal organisation sheet. It does not choose treatment, interpret a report or replace the clinic's written instructions. You can print it or copy the fields into your own notes; this page does not save your entries or contact your clinic.
Identify the visit
Appointment date: ____________________
Practitioner and clinic: ____________________
Where I keep the written visit summary: ____________________
The main question we discussed: ____________________
If you have not received the summary, note whom to ask for a copy. Keep the original alongside this sheet rather than trying to recreate it from memory. If your notes and the written instructions differ, ask the clinic to clarify the difference.
Turn the agreed plan into separate actions
Use one row for each administrative next step. An action might be booking the agreed review, requesting an existing document or sending a question through the clinic's confirmed channel. Record clinical instructions in the practitioner's own document, not as a new plan invented here.
| Agreed next step | Who will do it? | Date agreed with the clinic | Done / waiting / needs clarification |
|---|---|---|---|
A suggested action is different from a confirmed booking. Write “waiting for confirmation” where that is the actual position. If no date was agreed, write “ask clinic”; the blank does not mean you should choose a medical review interval yourself.
Keep the contact route beside the action
Clinic contact for administrative questions: ____________________
Confirmed route for a question about the plan: ____________________
Any response timing the clinic has actually stated: ____________________
What the clinic told me to do if I have not heard back: ____________________
Confirm the recipient before sharing a report or personal details. A contact found in an old message may no longer be the right route. This sheet makes no promise about the clinic's response time or whether somebody monitors messages outside working hours.
Record what still needs explaining
The instruction or term I want clarified: ____________________
The exact question I will ask: ____________________
Who I asked and when: ____________________
Their reply, or where I saved it: ____________________
You can write: “Please explain what I should do next and where those instructions are written.” Avoid filling an unclear instruction with something read online. If you need help sooner, contact a medical service; completing this sheet is never a reason to postpone care.
Confirm the next appointment
Is another appointment agreed, or am I waiting for advice? ____________________
Date, time and location or call link, once confirmed: ____________________
Who books it: ____________________
Documents the clinic asked me to bring: ____________________
Questions to carry forward: ____________________
If a referral is part of the plan, use the separate referral handover sheet for communication between practitioners. This action sheet keeps the tasks from this one visit together; it does not replace the referral itself.
Choose what to share with someone helping you
You may want help with a booking without sharing the whole consultation. Mark the task you want help with and the information needed for it.
Person helping, if I choose: ____________________
Task I want help with: ____________________
Information I choose to share: ____________________
Keep the completed sheet somewhere you are comfortable storing health information. For the next conversation, the appointment preparation guide helps you put your main question first. Yellow's resources support your preparation; the practitioner remains the source of your individual care instructions.
If you are exploring Yellow beyond these worksheets, you can read the programme information at Yellow for Hormones. This is an optional programme link, separate from your practitioner's instructions and any time-sensitive care.
This resource provides general information and does not diagnose a condition or replace advice from a qualified practitioner.
