Ask the clinic for the consultation fee, what it includes and what would be charged separately. Keep the reply beside the appointment details. This worksheet helps you collect a quote in rupees; it does not estimate what your care should cost or tell you which tests or treatments you need.
You can copy the questions into a message or use them on a call. You do not need to send a symptom diary or medical reports to ask about booking fees. If the clinic needs information to identify the right appointment type, ask what it needs and how to share it.
Start with the appointment you are booking
Name the appointment type before comparing fees. Write down whether you are asking about a first consultation, a follow-up or another service the clinic has described. Keep the practitioner's name and the location with the quote so you can refer to the same booking later.
You might write:
Hello. I would like to ask about a first appointment to discuss changes in midlife. Please confirm the consultation fee, what that fee includes, and whether there are separate registration or booking charges. Could you also share the cancellation and rescheduling terms?
For an online appointment, add:
If there is a connection problem, whom should I contact, and what happens to the appointment or payment?
These are questions for the clinic. An online listing or an old message may not answer them for the appointment you are about to book.
Put confirmed amounts and unanswered questions in separate columns
Leave an unknown amount blank. A blank is a reminder to ask; it is not a zero.

Keep your own travel, parking or time-away-from-work notes separately. They are part of planning your day, not charges from the clinic. If someone is accompanying you, include only the arrangements you have agreed with them.
Ask about a proposed test before adding it to the plan
A booking checklist is not a shopping list for investigations. If a practitioner proposes a test during the consultation, ask them what question it would answer and how its result would affect the next step.
Once the purpose is clear to you, you can ask where it can be done and request the relevant fee information. If you are quoted a package, ask for its contents and charges in writing. Do not treat the number of items in a package as a measure of how appropriate it is for you.
This page cannot decide whether a test is needed. That conversation belongs with the practitioner who is assessing you.
Make the follow-up arrangements explicit
Before paying for another appointment, check what you are booking it for. Is it a conversation about results, a review of a plan, or a new concern? Ask the clinic how it handles that type of appointment and record its reply.
If you intend to seek reimbursement, ask your insurer or employer's benefits administrator about your own policy and the documents required. A clinic's invoice is not, by itself, confirmation that a claim will be paid. Do not enter an assumed reimbursement as money already available in your notes.
Write any question about affordability in your appointment brief. You could say, “I would like to understand the costs of the proposed next steps before we agree on the plan.”
Keep the quote with the booking confirmation
Save the date, appointment location or joining instructions, the confirmed fee and the clinic's contact channel together. Keep receipts after payment. Check that you can find these details without searching through an entire message history.

Start with the unanswered line in your fee table. Ask the clinic to confirm it before you rely on the amount.
